Documents to be presented for the extradition of the body

To transfer the body abroad it is necessary:

  • request authorization to leave the country from the mayor of the municipality of death;
  • receive the ASL certificate of compliance with health laws;
  • obtain the death certificate and authorization from the embassy for the introduction of the body into the country to which it is headed.

Authorization for the introduction of the body

To introduce a body from abroad into Italian territory, an authorization is required, called a mortuary passport, which is issued by the competent authority of the place of origin. This procedure is valid for states adhering to the Berlin Convention. For non-member countries, the interested party must submit a specific application to the Italian consular authority abroad accompanied by a certificate from the competent local health authority and other documents and declarations.

Death occurred at home?

If the death occurs at home, it is necessary to contact the general practitioner or the 118 doctor to ascertain the causes of death. He will complete the necessary documents: death declaration, medical certificate and Istat card, to be sent to the Statistics Office of Rome Capital. Within 24 hours following the death, the death must be notified to the Municipality’s registrar, sending the documents drawn up by the doctor and the personal data of the deceased.

Death occurred in hospital?

When the death occurs within a healthcare facility, the death declaration, the medical certificate and the Istat card are drawn up by the healthcare management of the facility. They must then be sent to the Civil Status Officer of the Municipality as in the case of death in the home.

Request for authorization for the cremation of the body

If the deceased has expressed the desire to be cremated in his/her will or has joined a cremation association, it is possible to proceed with the formal request.

 

The request for cremation must be presented to the Cremation Office of AMA S.p.A. Capitoline cemeteries accompanied by the following documents:

  • Medical necropsy certificate;
  • Declaration in lieu of an affidavit, made by the spouse or closest relative, pursuant to the Presidential Decree. 445/2000 with related valid document;
  • Declaration of conformity of the coffin, pursuant to art. 47 Presidential Decree 445/2000;
  • Receipt certifying payment of investigation fees.
